Forms to Return to School:

• Affirmation of Isolation - For students returning to school after testing positive

Testing Overview

Symptomatic Students:

• If a home test is negative, but the student is displaying symptoms, they must get a negative PCR test to return to school
• If a home test is positive, that is accepted by the school and needs to be registered here

Asymptomatic Students:

• If a home test is positive, the student cannot come to school and their test should be registered here

Symptomatic Testing -

If a student comes to school and develops symptoms during the school day, you will have the option of having them tested before they are sent home. To register for this, click here.
